JOB SUMMARY

Under the supervision of the Pharmacy Manager and within a collaborative team environment, the staff pharmacist reviews, compounds, and dispenses both IV and oral medications utilizing standard physical and chemical procedures to fill medication requests insured by physicians and other qualified prescribers. Receives and reviews medication orders to verify the appropriateness of doses regarding patient’s age, weight, and allergies. Screens for potential drug interactions, documents interventions, reviews and updates medication reconciliation duties as deemed necessary. The pharmacist is accountable for the timely achievement of individual and team metrics as assigned. The pharmacist also instructs and supervises Pharmacy Technicians and Interns.


ESSENTIAL JOB FUNCTIONS

  • Delivers direct patient care and clinical practice, including decentralized and service-based programs 
    • Works as an active member of multidisciplinary team and collaborates with healthcare providers within decentralized patient care areas to provide patient-centered care 
    • Identifies high-risk patients and implements interventions to improve quality and safety 
    • Makes appropriate evidence-based, patient-centered medication recommendations 
    • Participates in the management of medical emergencies 
    • Provide discharge medication review, reconciliation, and counseling as appropriate 
  • Performs renal functionality review, aminoglycoside/vancomycin dosing and monitoring, antimicrobial stewardship functionality, anticoagulation monitoring/management, and any other clinical pharmacist duties as necessary.
  • Compounds extemporaneous products using correct techniques and formulas. Prepares and affixes labels to containers showing identification data and directions for use as required.
  • Prepares, supervises and dispenses non-hazardous and hazardous sterile compounds in accordance to USP 797, USP 800 requirements and Wyoming Board of Pharmacy Chapter 17 requirements. 
  • Maintains required home IV records, documentation, dispensing and billing as related to Hospice patients.
  • Oversees all schedule II orders and deliveries to the respective areas.
  • Supervises and inspects all prepackaging procedures and filled requisitions completed by the Pharmacy Technician.
  • Provides drug information to physicians, nurse staff, patients, and other staff as needed. Mentors students and assists in educational duties as necessary.
  • Oversees and reviews unit dose packaging, IV admixture, and Drug Use Evaluations (DUE).
  • Actively participates in Continuous Quality Improvement for the department and organization. Must meet all education requirements of the Hospital and Pharmacy (pharmacist’s competency checklist).
  • Responsible for the maintenance and proper operation of equipment within the department.
  • Must be free from governmental sanctions involving health care and/or financial practices.
  • Performs other duties as assigned.
     

POSITION QUALIFICATIONS

Education / Experience / License and Certifications

  • Registered Pharmacist in the State of Wyoming, keeps license in good standing.
  • Bachelor of Science in Pharmacy or Doctorate of Pharmacy degree.
  • ACLS certification, preferred.
  • BLS certification, preferred.

Additional Skills

  • Able to communicate effectively in English, both verbally and writing.
  • Good interpersonal skills are needed to work with staff and physicians.
  • Strong written and verbal skills
  • Analytical, interpretive skills, requiring higher levels of short and long-term mental faculties accompanied by long and short-term memory.
  • Strong computer knowledge
  • Hand/eye coordination, fine and gross motor skills.
  • Able to prioritize multiple details simultaneously, able to troubleshoot problems.
  • Experience in institutional practice is highly desirable. 
     

Specific demands not listed: Possible exposure to blood and or body fluids / infectious disease / hazardous waste requiring the use of Personal Protective Equipment. Exposure to odorous chemicals / specimens and Latex products.
