JOB DESCRIPTION
 
Title: Licensed Practical Nurse (LPN)
Location: 211 Smith Street, Sheridan, WY 82801
Reports to: Help at Home Clinical Manager
 
ESSENTIAL JOB FUNCTIONS
 
Patient Care Responsibilities
Assess patients’ health condition and chart observations at each visit.
Record patients’ vital signs and medical information.
Administer medications, fill weekly medication boxes, and other personalized interventions as prescribed by a physician.
Monitor patients’ symptoms and communicate progress to a physician.
Offer advice and emotional support to patients and their family members.
Teach and educate patients and their families on managing illnesses or injuries.
Help with personal hygiene, daily activities, and homemaking.
Coordinate with doctors, other healthcare professionals, and RNs to create and evaluate customized care plans.
Safety and Compliance
Maintain a safe and clean working environment in compliance with healthcare procedures and regulations.
Report health and safety concerns regarding an individual or the program to a supervisor immediately.
Complete orientation and ongoing training as directed.
Professionalism and Confidentiality
Maintain confidentiality.
Communicate and work with individuals in a respectful manner.
Additional tasks and responsibilities as needed to support the organization's goals and operations.
Knowledge, Skills, and Abilities
Coachable with a willingness to learn and adapt to a changing workflow.
Comprehensive understanding of patient care, including medication administration and chronic disease management.
Proficient in recording and managing medical information accurately and efficiently.
Teaching and coaching skills to educate patients and families about managing health conditions.
Organizational and time management skills to manage multiple tasks and prioritize effectively.
Ability to work collaboratively with healthcare professionals to ensure comprehensive patient care.
Demonstrated ability to provide compassionate, patient-centered care while maintaining professional boundaries.
Physical Demands
Access to reliable transportation for traveling between work locations.
Ability to move within a variety of settings, including patient homes and office spaces.
Occasionally lifting or moving items, such as medical supplies, or assisting patients with transferring 
Adequate hand-eye coordination and fine motor skills to perform tasks such as medication administration and using medical tools or equipment.
Clear verbal communication for patient and team interaction, as well as the ability to document information accurately.
Ability to read medical charts, observe patient conditions, and listen effectively to verbal cues or instructions.
The position may require extended periods of focus or activity, but rest breaks are supported and encouraged as needed.
Minimum Qualifications
Licensed Nurse with a current license in the State of Wyoming.
Valid driver’s license with a safe driving record.
Ability to provide a clean, current Motor Vehicle Department (MVD) report.
Employment contingent upon passing a Central Registry background check.
Employment contingent upon passing a Division of Criminal Investigations background check.
One year of experience in a home health setting (preferred).
Experience working with older adults or people with disabilities (preferred).
