JOB TYPE - Contract position 24 hrs a week (potential of growing into regular full-time if desired)
 
LOCATION – Sheridan, Wyoming
 
POSITION SUMMARY
This role is responsible for providing medically prescribed, goal-oriented physical therapy in a patient’s home or select location based on treatment/patient needs. The role includes providing therapeutic treatments and participating in the development of patient physical therapy plans of care.
 
PRINCIPAL ACCOUNTABILITIES
  • Provide skilled progressive treatment to patients normally ranging from 60 to 85 years of age.
  • Develop treatment plans and deliver treatment in the areas of balance/fall prevention, post-op care, orthopedic/sports medicine, spine dysfunction and preventative care.
  • Coordinate manage and schedule appointment with patients.
  • Complete all required medical and agency documentation.
  • Participate in team meetings to coordinate quality patient care, exchange information. problem solve and receive team support.
 
QUALIFICATIONS
  • Minimum three years clinical experience.
  • Compassionate, creative and able to deliver quality patient-centered care.
  • Excellent problem solving, interpersonal and communication skills.
  • Motivated and able be excel in a self-managed environment.
  • Desire to work a flexible schedule based on patient needs.
  • Ability to travel to patient homes or desired location for treatment.
 
EDUCATION - Minimum bachelor’s degree in Physical Therapy from accredited PT program
 
CERTIFICATIONS, LICENSES AND REGISTRATIONS
- Currently licensed Physical Therapist in the state of Wyoming.
- Possesses and maintain a current CPR certification
- Valid driver’s license and auto liability insurance
