This position works 24-29 hours per week, Monday through Friday. Rate of pay is $30 per hour DOE. Must be currently licensed as a Registered Nurse in the State of Wyoming. Benefits may include paid time off, vision, dental, and Wyoming Retirement.

JOB DESCRIPTION
POSITION: Registered Nurse
LOCATION: Throughout Sheridan County
REPORTS TO: Director of Help at Home


Essential Job Functions Include:
• Demonstrates positive leadership by upholding organizations cultural values, vision and mission
• Provides one-on-one assistance for clients that promote independence, quality of life and dignity - including but not limited to information, intake, evaluation, plan of care development and implementation when applicable, case supervision when applicable, service termination when applicable, follow-up and quality assurance
• Provides one-on-one nursing care as needed - may include medications set up, personal care, and other needs as determined by plan of care
• Performs in home nursing assessments as required by program directives
• Provides supervisory in-home visits
• May assist individuals with Activities of Daily Living & Instrumental Activities of Daily Living in the home setting
• Provides mentorship and coaching for home health aides, homemakers, and Day Break Certified Nursing Aides
• Conducts supervisory review of Day Break Certified Nursing Aides
• Evaluates and responds appropriately to individual client needs and concerns
• Communicate client welfare in a timely manner to program director, and clients care team
• Communicates and works with individuals in a respectful manner - frequently communicates and exchanges information with a wide variety of people with different personalities and needs
• Performs TB risk assessments for appropriate Hub employees annually
• Collaborate with program director to provide team development training
• Be knowledgeable of state and federal programs including Wyoming State Aging programs, state grants, Medicaid Long-Term Care Waiver program and Veteran’s Administration
• Maintains confidentiality observing HIPAA and HITECH protocol
• Completes orientation and on-going training as directed
• Assists Program Director in the development and implementation of ongoing staff training and development
• May be asked to provide public relations information for aging well – including writing articles and public speaking
• Networks with other community organizations
• May be on-call evenings and weekends on intermittent basis
• Other duties as assigned

Physical Demands:
• Transfers, lifts, and positions objects up to 50 pounds
• Drives to the home setting, using fleet car or as needed provides own transportation

Special Knowledge, Skills and Abilities:
• Experience working with older adults and people with disabilities in the home setting and their families
• Ability to effectively communicate, both orally and in writing

Minimum Qualifications:
• Licensed Nurse with current license in the State of Wyoming, with one year of experience in a home health setting required.
• Experience working with older adults or people with disabilities preferred.
• Must have a reliable vehicle, a valid driver’s license, insurance, and a safe driving record.
• Employment dependent on ability to pass a Central Registry and Division of Criminal Investigations background check and provide a clean, current Motor Vehicle Department Report.
